An “Evidence of Coverage” (EOC) is a document sent annually by your plan each fall that lets you know what the plan covers, how much you pay, and more. This document can help you to review changes to your plan and to decide if a plan will continue to meet your needs in terms of costs and benefits. The document is typically sent in September of each year.
Keep an eye out for your Annual Notice of Change (ANOC) document as well to help you better understand any changes made to your plan.
